- W1982581336 abstract "Abstract Introduction: Recent studies have shown that laparoscopic inguinal hernia repair (LIHR) complicates, if not contraindicates, subsequent radical retropubic prostatectomy (RRP) due to fibrotic obliteration of the space of Retzius. This raises concern over treating men post-LIHR who develop prostate cancer (PCa). We reviewed recent LIHR candidates for elevated PSA or biopsy-proven PCa to assess the role of screening these patients prospectively for PCa. Methods: Between January and September 2003, 137 consecutive male patients underwent LIHR at our institution. All patients were counseled to have PSA drawn before surgery. We excluded 31 patients who had no PSA drawn and 3 others with previous known history of PCa. Urological evaluation and transrectal ultrasound (TRUS)-guided biopsies were recommended for patients with PSA ≥ 2.5, levels denoting definite increased PCa risk. Additionally, we evaluated patients with PSA 1.0–2.5, levels possibly indicating developing PCa. Results: Of 103 eligible patients, median age was 53.9 years. Twenty-one patients (20.4%) had PSA ≥ 2.5. Furthermore, 28 patients (27.2%) had PSA 1.0–2.5. Overall, five patients (4.9%) were diagnosed with prostate cancer (four patients) or high-grade prostatic intraepithelial neoplasia (one patient). Conclusions: Patients at risk for PCa should be counseled regarding the difficulty and potential complications of RRP following LIHR. Prostate cancers could then be addressed before the hernias. As urological evaluation was advised for 47.6% of patients, our institution has implemented a PCa surveillance program before LIHR. We recommend screening for all patients over 30 years old and TRUS-guided biopsy for any suspicion of PCa." @default.
