- W2894565259 abstract "Introduction: Postoperative pancreatic fistula (POPF) diagnosis is based on amylase level on drainage fluid. There are no blood markers allowing early diagnosis of POPF. C-Reactive protein (CRP) is useful for diagnosis of anastomotic fistula after colic resection. The aim of this study was to assess the value of CRP for the diagnosis of clinically relevant POPF (CR-POPF). Methods: Post-operative outcomes were collected retrospectively, including POPF occurrence and CRP values until POD 6. POPF was graded according ISGPF-2005 and CR-POPF included grade B and C. Overall, 345 pancreaticoduodenectomies (PD) and 216 distal pancreatectomies (DP) were included. Results: After PD, 95 (27.5%) patients had a CR-POPF. CRP value at POD 1-2 was higher when CF-POPF occurred (194 ± 75 versus 150 ± 65mg/L, p = 0,002). Even when patients developed CR-POPF after POD 7, CRP value at POD 1-2 was higher (181 ± 71 versus 143 ± 64mg/L, p = 0,04). The difference remained significant at POD3-4 and thereafter. Fourty (18.5%) patients developed CR-POPF after DP. POD1-2 CRP value was higher in patients that had CR-POPF compared with patients that had no CR-POPF (156 ± 90 versus 103 ± 70mg/L; p = 0,002). There was no significant difference in CRP value after POD 2. At POD 1-2, PPV was 81% after PD and 90% for the diagnosis of CR-POPF for a CRP value >200mg/L. CRP value <100mg/L at POD 1-2 excluded CR-POPF with NPV of 89% after PD and 71% after DP. Conclusion: CRP measurement at POD 1-2 may be useful for predicting CR-POPF particularly after PD. Some prospective studies should be conducted to confirm these primary results." @default.
