- W562500973 abstract "大動脈弁閉鎖不全症を合併する大動脈弁輪拡張症に対し，Prima Plusステントレス生体弁をFull root法で使用し大動脈基部置換術を施行したが，移植65カ月後にブタ大動脈壁破裂による仮性動脈瘤を形成し，再手術を要した症例を経験した．症例は63歳，女性．術後問題なく経過していたが，心不全症状が急速に進行した．3D-CTで大動脈基部に仮性動脈瘤の発生を認め，心エコー検査で重度の僧帽弁閉鎖不全症を認めた．再手術時に，Prima Plus生体弁の無Valsalva洞壁に約2 cmの横方向の亀裂が存在しており，仮性動脈瘤の原因となっていた．また，僧帽弁は両弁尖の肥厚短縮を認めた．機械弁を使用した再大動脈基部置換術と僧帽弁置換術を施行し，術後は良好に経過した．ブタ大動脈壁の病理組織で，宿主の単核細胞・単球およびマクロファージの浸潤が認められ，破裂の原因として免疫反応の関与が疑われた．ステントレス生体弁は血行動態的に優れた人工弁であるが，full root法で使用した場合の合併症として，ブタValsalva洞の拡大と破裂の報告が散見される．心エコー検査やCTによる綿密な経過観察が必要である．" @default.
