Plaintiff filed 180 requesting injunctive relief and 188 , alleging Defendants' unlawful conduct. On 10/30/17, the Court took 180 under advisement, ordering Defendants to file a response and granting Plaintiff's request for time to reply. The following day Plaintiff filed another emergency motion for medical attention 197 . He claims that, though Dr. 0525C7E renewed his Ultram prescription, he is not receiving it and is in debilitating pain. Defendants have filed a joint response to 180 and 197 , providing medication records which show that Plaintiff has been prescribed Ultram for pain, two tabs daily at bedtime. Defendants have produced medication records which show that Plaintiff received Ultram uninterruptedly every evening in June, July, August, September and October 2017. The records contain the initials of various Pontiac medical personnel who administered the Ultram. Defendants also provide the affidavit of Dr. 0525C7E, the Pontiac Medical Director, who is also named in this case. Dr. 0525C7E refers to the records and attests that on 7/26/17, Plaintiff received a 3-month renewal of his Ultram prescription by Physician's Assistant Ojelade. Dr. 0525C7E saw Plaintiff on 10/12/17 and renewed the Ultram for six months, prior to the previous prescription lapsing. On 10/16/17, Plaintiff was seen by Nurse Practitioner Cheryl Hansen for a routine exam. On 10/19/17, Plaintiff underwent an x-ray to his right knee. On 10/29/17, he underwent a rectal examination. Dr. 0525C7E disputes that Plaintiff has not been provided care when requested. He notes that the Ultram prescription currently remains in effect through April 2018. Dr. 0525C7E also indicates that Plaintiff has been writing to him, with requests for treatment, rather than submitting a sick call slip per protocol. Plaintiff has written, asking that his medication renewals be changed so that all of his prescriptions expire at the same time.
