The Court has reviewed the proposed amended complaint under 28 U.S.C. § 1915A, and found that Plaintiff has stated a claim for deliberate indifference to a serious mental health condition. Sanville v. McCaughtry, 266 F.2d 724, 734 (7th Cir. 2001). The Clerk is directed to issue summonses to Defendants Dr. 1BF518E and 0ADED18, who Plaintiff describes as a liaison to Dr. 1BF518E, and the U.S. Marshal is directed to serve them. Any service forms necessary for Plaintiff to complete will be sent by the Marshal as appropriate to serve Defendants with process. The U.S. Marshal is directed to make all reasonable efforts to serve Defendants. With respect to former correctional employees who no longer can be found at the work address provided by Plaintiff, the Will County Adult Detention Facility shall furnish the Marshal with Defendant's last-known address. The information shall be used only for purposes of effectuating service [or for proof of service, should a dispute arise] and any documentation of the address shall be retained only by the Marshal. Address information shall not be maintained in the court file, nor disclosed by the Marshal. The Marshal is authorized to mail a request for waiver of service to Defendants in the manner prescribed by Fed. R. Civ. P. 4(d)(2) before attempting personal service. Plaintiff must fill out the paperwork as completely as possible and return it to the Marshal in a timely fashion. If Plaintiff is able, he should identify the last name of Defendant 0ADED18, in order to ensure that she is properly served. The Court notes that Counsel for Defendants has filed a motion to dismiss Defendant 3C0D010, and that the motion is now fully briefed. The Court will rule on the pending motion by mail.
