Plaintiff's in forma pauperis application 3 is denied without prejudice because it does not include the necessary trust fund ledgers. In addition, Plaintiff's complaint is facially deficient because he did not use the court's mandatory form. N.D. Ill. L.R. 81.1. Thus, the complaint 1 is dismissed without prejudice. If Plaintiff wants to pursue this case, he must: (1) pay the full statutory filing fee of $400 or submit an application for leave to proceed in forma pauperis that is certified by a correctional official and accompanied by a copy of his trust fund account statement showing all transactions in his account for the six-month period immediately preceding his initiation of this action (in other words, from May 18, 2020, to the date of compliance); and (2) submit a signed amended complaint on the court's form. If Plaintiff does not comply with this order by 1/19/2021, this case will be summarily dismissed. The Clerk is directed to send Plaintiff a blank application for leave to proceed in forma pauperis for incarcerated persons, an amended complaint form, filing instructions, and a copy of this order.
