His allegations, however, are vague, and it is not clear whether he intends to pursue a lawsuit. Additionally, he has been transferred to Stateville Correctional Center, according to the docket, or to Menard Correctional Center, according to the IDOC website. If Plaintiff seeks to file a lawsuit about his lack of care at Pontiac, he must name as defendants the individuals who were aware of and disregarded Plaintiff's serious mental health needs. Additionally, Plaintiff must have exhausted all of his available administrative remedies within the prison before pursuing a lawsuit, which means he must have filed a grievance and pursued all possible appeals. If Plaintiff did not intend to file a lawsuit, than Plaintiff may file a motion to dismiss this case by January 6, 2017, and the filing fee assessment will be vacated. If Plaintiff does intend to pursue a lawsuit, Plaintiff is directed to file a complaint naming the individuals at Pontiac who personally denied Plaintiff necessary mental health treatment and explain how they did so. The clerk is directed to send Plaintiff a form complaint with instructions. If Plaintiff does not respond to this order by January 6, 2017, this case will be dismissed, without prejudice.
