See 28 U.S.C. 1915(a)(1). The Court reminds plaintiff that her statements on this application are a sworn affidavit and, if shown to be false, plaintiff's case may be dismissed. See Mathis v. New York Life Ins., 133 F.3d 546, 547-48 (7th Cir. 1998). Plaintiff's motion for appointment of counsel 5 , however, must be denied at this time. Plaintiff must attempt to retain counsel to represent her in this case and document those attempts to the Court before a motion for appointment of counsel can be considered.
