If Plaintiff wants to proceed with this case, he must either (1) prepay the $400 filing fee; or, no later than October 1, 2019, (2) file a renewed in forma pauperis application in which he explains, under oath and with supporting records, why he is unable to pay the filing fee. Plaintiff's statement must include one-years' worth of trust fund account statements, and information setting forth the source of the moneys he received during the six months prior to initiating this action, how that money was spent, and why those moneys should not be counted against his ability to pay the filing fee. If Plaintiff decides to pay the $400 filing fee, he may do so by sending a checkor money order made payable to Clerk, United States District Court. Any payment should be sent to the Clerk, United States District Court, 219 South Dearborn Street, Chicago, Illinois 60604, attn: Cashier's Desk, 20th Floor, and shall clearly identify Plaintiff's name and the case number assigned to this case. Failure to comply with this order by the date stated above, will result in summary dismissal of this case. The Clerk is directed to send Plaintiff a blank application to proceed in forma pauperis and a copy of this order. The Court defers initial review of the proposed complaint 1 pursuant to 28 U.S.C. § 1915A until Plaintiff complies with this order.
