- W103285983 abstract "1826 Objectives To assess the value of dual time point FDG PET/CT in the differentiation of extrahepatic biliary malignancy from benign disease, and the potential benefits of delayed PET/CT image. Methods Thirty six patients with suspected extrahepatic biliary malignancy based on conventional imaging studies such as ultrasonography, CT and MRI between July 2008 and October 2010 were included. All patients underwent dual time point PET/CT scans at 1 hour and 2 hours after radiotracer injection. From the site where malignancy was suspected on conventional imaging studies, SUVmax for both time points (SUVmax1 and SUVmax2) and the percent change in SUVmax (Δ%SUVmax) between the two time points were calculated. The ratio of SUVmax1, SUVmax2 and Δ%SUVmax in comparison with average SUV of right hepatic lobe were generated (T/L ratio1, T/L ratio2 and Δ%T/L ratio). PET/CT results were correlated with histological results and radiological follow up for at least 6 months. Results Of 36 patients, the final diagnosis was malignancy in 31 patients and benign disease in 5 patients. For all patients, SUVmax2 was higher than SUVmax1 (p=0.010). Between malignant and benign lesions, there were significant differences in SUVmax1 (5.7±4.8 Vs. 2.3±0.8, p=0.003) and SUVmax2 (6.3±5.4 Vs. 2.3±0.7, p=0.001). There was no significant difference in Δ%SUVmax, T/L ratio1, T/L ratio2 and Δ%T/L ratio. On ROC analysis, SUVmax1 with cut-off value of 3.3 demonstrated sensitivity of 74.2%, specificity 100% and accuracy 77.8% and SUVmax2 with cut-off value of 3.1 demonstrated sensitivity 93.5%, specificity 100% and accuracy 94.4%. Conclusions SUVmax from the dual time points are both useful parameters in the differentiation of extrahepatic biliary malignancy. Though SUVmax from the 2 hour image had higher accuracy, there was no statistically significant difference compared to the initial 1hour parameter" @default.
