- W109503463 abstract "症例は37歳，女性．腰痛を主訴に近医を受診．腰椎X線検査にて偶然，下肺野に腫瘤影を認めたため，当院呼吸器外科に紹介された．胸部X線写真で左下肺野に約9 cmの境界明瞭円形の腫瘤陰影を認め，胸部CTおよびMRIにて第9椎間孔から椎体を破壊し脊柱管内に伸展する，ダンベル型の腫瘍を認めた．腫瘍は脊髄硬膜内には伸展しておらず，中枢に近い肋間神経から発生した神経鞘腫と考えられた．手術は一期的にまず後方より神経根を処理した後，体位変換し前方アプローチとしては10 cmの第5肋骨床開胸，胸腔鏡併用で開始した．しかし腫瘍径が大きく，腫瘍の強度の広範癒着のため胸腔内操作に難渋し，胸腔内操作にもう一ヵ所の開胸創を要した．術後経過良好で，腰痛は軽快した．ダンベル型腫瘍に対する胸腔鏡補助下の後方＋前方アプローチは美容や安全性の面から導入すべき手技であるが，胸腔鏡の役割は補助的な使用にとどめるべきであると考える．" @default.
