- W11118686 abstract "Glycohemoglobin (GHb) has a key role in the assessment of glycemic control in diabetic patients. Several studies have clearly shown that improved glycemic control is strongly associated with decreased development and/or progression of diabetic microvascular complications in both type 1 and 2 diabetes mellitus. Therefore accurate determination of GHb concentration is an important issue for clinical laboratories. Several factors may affect the results and lead to erroneous results. This study analysed the effect of analytical factors, pathologic conditions and drugs on HbA1c results. In a first study we related that labile HbA1c fraction, if not adequately separated, is a positive interference on final HbA1c results. In this cases a pre-treatment to removal this fraction is needed. Storage condition is also an important pre-analytical factor. Samples kept at 4C are stable for ten days and storage at –80C is preferred for longterm storage. Sample stored at –20C showed statistically significant lower HbA1c concentration by one day of storage. In another study we related that the presence of anomalous hemoglobin is associated with very low values of HbA1c. Additionally, we demonstrated that anemia is an important source of negative interference. Hematological status should always be considered to ensure the correct interpretation of GHb results. In a third study we investigate the effect of usual doses of aspirin, vitamin C and E on HbA1c levels. These drugs do not affect HbA1c results measured by three different assays, two HPLCs and one immunoassay, in a group of non-diabetic volunteers. Clinicians must be aware of all pitfalls to avoid adding more confusion to the clinical interpretation of HbA1c values and discrepancies between clinical impressions and laboratory data should be always investigate." @default.
