- W11676989 abstract "Reactive airways dysfunction syndrome (RADS) refers to asthma that begins after exposure to a very high level of respiratory irritant (usually accidental). The initial criteria included the onset of asthma symptoms within 24 h of a single such exposure, absence of preceding lung disease, objective evidence of asthma (spirometry showing a significant bronchodilator response, or methacholine or histamine challenge showing significant airway hyperresponsiveness), and continuation of asthma symptoms for at least three months. The criteria for this syndrome have been variably expanded to include multiple irritant exposures, less massive levels of exposure, a more delayed onset of symptoms after exposure, or a shorter duration of symptoms, termed ‘irritant-induced asthma’. RADS is considered a subset of this more inclusive, but less well defined entity. The diagnosis of RADS is largely circumstantial, relying on the history of exposure, absence of documented preceding airway disease, and objective documentation of asthma. Airway pathology does not clearly distinguish this syndrome from other asthma. Therapy is the same as for other types of asthma including corticosteroids and bronchodilators, as well as evaluation of workplace exposures.
