J Pediatr 1963;63:315-30 This is a very interesting manuscript that reports the clinical findings and the medical and surgical management of 9 children with pheochromocytoma and reviews the literature on 91 additional previously reported cases of pheochromocytoma in children. Although hypertension in patients with a pheochromocytoma is frequently thought to be intermittent, the authors found that hypertension was sustained in 88% of patients. In addition, the authors found that multiple tumors were common and occurred in 32% of children and that bilateral adrenal tumors and were found in 23% of patients. Extra-adrenal tumors were present in 31% of the children, and today it continues to be recognized that multiple tumors and extra-adrenal tumors are more common in children than in adults. The diagnosis was usually made by determining epinephrine and norepinephrine levels in the urine, and urinary analysis of catecolamines and catecholamine metabolites continues to be a common way to screen and diagnose pheochromocytoma in children today. The tumors were visualized most commonly by retroperitoneal pneumogram and fortunately today these tumors are visualized by newer technologies including computed tomography and magnetic resonance imaging scans. The mortality rate was high and was 45% before 1954 and 13% in later years; the authors attributed the decreased mortality during the later years to the availability of better pharmacologic agents including regitine and norepinephrine for blood pressure control. Fortunately, today there are many more pharmacologic agents available for preoperative and intraoperative management of blood pressure in children with a pheochromocytoma. Although rare in children, pheochromocytomas continue to be a form of secondary hypertension that is curable in children." @default.
