• W1264967557 abstract "OBJECTIVE: For quality improvement, we analyzed time delays in patient placement from the Emergency Department (ED) to the hospital floors with a focus on Neurology patients admitted to an intensive care unit (ICU) or step-down unit (SDU) setting. BACKGROUND: Patient admission and placement into their appropriate level of care is a critical step in ensuring proper patient care. Delays result in patients spending an extended period of time in the ED receiving medical care in a less than ideal environment. DESIGN/METHODS: For every patient that was admitted to the Neurology Service over a 6-month period, those that had spent greater than 12 hours in the ED were recorded into a database. In addition to the amount of hours spent in the ED, their level of care was documented along with the reason behind the time delay in patient placement. Once the primary reason was determined, the subsequent 6 months focused on ways in which these delays could be avoided. RESULTS: Over a 6-month period, a total of 18 patients, all ICU and SDU level of care patients, spent >12 hours (maximum 72 hours) in the ED awaiting placement. With fourteen of the cases, the reason behind the delay was directly due to bed availability in a higher-level unit. In several instances however, it was noted that patient beds were in fact available, unbeknownst to the ED nurse manager. A system was put into effect amongst the Neurology residents that allowed for direct resident-to-resident communication regarding all available ICU and SDU beds in an effort to avoid such delays. The subsequent 6 months have shown 2 Neurology patients (89[percnt] reduction) spending >12 hours in the ED. CONCLUSIONS: When discussing the need to utilize a high level of care patient bed, direct communication amongst residents allows for more accurate assessment and utilization of available hospital resources.
