- W126861805 abstract "1654 Objectives: Effective locoregional therapy of liver metastases, such as partial liver resection (PLR) or radiofrequency ablation (RFA), requires accurate delineation of malignant tissue. Functional imaging with FDG-PET provides a tool for visualization of liver metastases, but the technique suffers from uncertainties related to the relatively poor spatial resolution and not-straightforward thresholding. The aim of the study was to determine an optimal algorithm for size determination of liver metastases on PET. Methods: 10 patients with 13 evaluable liver metastases from colorectal carcinoma underwent FDG-PET of the liver prior to laparotomy. The true largest tumor diameter was determined at pathological examination for PLR specimens and by intraoperative ultrasound and palpation for RFA lesions. On PET images, tumor delineation and subsequent mathematical determination of the largest tumor diameter was performed using several treshholding techniques: Different standard uptake values (SUV), different percentages of the maximum lesion intensity (LI) or the average background intensity (BI), and by application of na iterative relative thresholding level (RTL) that corrects for the influences of lesion size and intensity. Results: 7 metastases were resected and 6 underwent RFA. True largest diameters ranged from 15 to 80 mm. Many different SUV, BI and BL levels failed to generate adequate delineation in one or more cases. Succesful delineation in all cases with optimal size measurement was achieved with SUV=4.0, LI=60%, BI=140%, and RTL, with average deviations in the measured largest lesion sizes of 9, 7, 8 and 7 mm, respectively. There was no significant difference between any of the succesful methods. Conclusions: The size of liver metastases from colorectal carcinoma can be determined using PET with an average accuracy in the range of the image resolution (7 mm FWHM). Multiple delineation thresholds are suitable, but the iterative RTL method seems preferable on theoretical grounds as it is independent of lesion size and intensity. The well-known thresholds SUV=2.5 and LI=50% were inadequate for this specific use." @default.
