- W128779322 abstract "The incidence of sudden coronary death is high in the first 6 months after myocardial infarction, amounting to 12–15% or more depending on the location and extent of infarction, the underlying coronary disease as well as the patient’s age. Its mechanism, re-entry tachycardia in the area of scar tissue leading to ventricular tachycardia and fibrillation, provoked by PVCs, is well established today. Prevention was tried by various means, especially the administration of class I antiarrhythmic drugs and betablockers. Only some of the latter group proved to be successful, especially alprenolol, practolol, timolol, propranolol and metoprolol as shown in randomized studies. In Europe a multicenter double-blind, randomized, placebo-controlled study was performed with oxprenolol slow release 160 mg b.i.d. The study comprised 1,741 patients who were followed for 12 months. There were 57 deaths (6.6%) in the oxprenolol group (OX) and 45 (5.1%) in the placebo group (PL), the difference being insignificant. Some special features might explain some of the reasons for the ‘negative’ outcome of the study with regard to prevention of sudden coronary death. 275 patients in each group discontinued the trial medication; mortality in these patients was higher for OX (13.1%) than for PL (7.6%) even weeks and months after withdrawal. Mortality was also higher in patients 65–69 years, 16.7% for OX and 5.6% for PL. In the 1,472 patients less than 65 years mortality showed a tendency to be lower in the 478 patients at high risk, 6.9% under OX versus 10.2% under PL. Hence, the EIS failed to confirm previous positive results in this field.
