- W130548610 abstract "Every severe burn injury with an extent of more than 15% of the body surface can lead to an acute danger to life because of burn shock. Within a very short time after the burn trauma, appreciable amounts of fluid are lost via the rapidly developing burn edema as well as via the wound surfaces [1]. The losses of plasma, erythrocytes, water, and salts give rise to a vicious circle: the colloid osmotic pressure falls, the extracellular sodium and water content is reduced, and there arise transmineralization processes, i.e., potassium leaves the cell in exchange for sodium ions, the viscosity of the blood increases. Disturbances of the microcirculation lead to a defective supply of the cells with oxygen and bring about development of a metabolic acidosis. The volume losses specified bring about a high secretion of catecholamines together with the pain, which results in a vasoconstriction and thus a marked rise in the total peripheral resistance. The individual factors are all closely interwoven in the course of the very complex pathophysiologic process and finally result in the full picture of burn shock without treatment corresponding to the requirements. The excessive extent of the shock causes it to differs significantly from all other forms of shock, because of the persisting large volume losses and the difficulty of making up the loss [1,3]. Under the conditions specified, it becomes clear that a timely substitution therapy adapted to the requirements must be of primary and crucial importance for the fate of a burn casualty. A supply of fluid beginning as soon as possible after the trauma must therefore be ensured, even in disaster situations." @default.
