- W13305756 abstract "More than a year ago, in a hotel conference room in New Orleans, the Generations Editorial Advisory Board members were tossing around themes for the 2011 issues of the journal. The subject of medications came up. Concerns were raised about the amount of medication being consumed by elders and the possible over-prescription of meds. One board member mentioned how medications can become a problem during care transitions: as people move across healthcare settings, they frequently receive layered-on prescriptions, which can spell trouble when taken in combination. The final consensus was that out of a long list of proposed topics, medication use in older adults was a critical area to explore. Medications can help prolong life, manage symptoms of chronic diseases, and improve quality of life and functioning for older but serious, even life-threatening, outcomes lurk if elders' multiple medications aren't carefully reviewed and monitored. This issue of Generations reviews the often controversial role of medications in the aging cohort, and frames the benefits and risks of medications, especially in fragile elders. Under the guidance of Guest Editor Thomas R. Clark, we offer a series of articles that addresses the basics of medication use in the elder population, along with a look at special challenges relating to medication use in this cohort: polypharmacy; adherence to drug regimens; the potential for medications to contribute to geriatric syndromes such as falls and delirium; and the vital role pharmacists play in delivering quality care. A pharmacist who worked as a consultant in long-term care before joining the American Society of Consultant Pharmacists (ASCP) in 1994, Tom Clark is ASCP's director of clinical affairs-a position that demands he stay current with medical and pharmacy literature on medication use in older adults. Because health professionals generally are not trained in principles of medication use in elders, medications are often used inappropriately in this cohort in a variety of ways, says Clark. the aging of the U.S. population, it is increasingly important that health professionals understand and apply these principles. Clark knows professionals in the field of aging are already aware that most elders take multiple medications, but one of his primary concerns is educating those who interact with older adults about the potential adverse effects medications can have on elders-effects like delirium that might be misinterpreted as a new medical condition. Clark says a number of recent books and media accounts have raised questions about the potential benefit and harm of medications in elders. And assumptions are often made about elders and their medication use, he says, but these assumptions are frequently inaccurate-plus, there is a paucity of research. Professionals in aging should be aware that clinical trials frequently exclude older making it difficult to know whether the benefits of a medication apply to this population, or whether this population might experience more frequent or severe adverse effects than younger adults, Clark says. With such cautions in mind, Clark intends this issue of Generations to highlight key issues and facts to help promote the appropriate use of medications in older people- especially frail elders. …" @default.
