- W137690900 abstract "Publisher Summary This chapter describes the indications for surgery in secondary hyperparathyroidism. Secondary hyperparathyroidism is a state of parathyroid overactivity induced by a fall in the circulating concentrations of ionized calcium and, possibly, 1,25-dihydroxyvitamin D3 . In such a state, it is usual for all four parathyroid glands to be hyperplastic, often asymmetrically. It is found that although adenomatous hyperplasia may be encountered, adenoma without hyperplasia is not a feature of secondary hyperparathyroidism. In patients treated by hemodialysis or peritoneal dialysis, the hyperparathyroid state continues and usually progresses despite treatment. At first glance, this is surprising because it is generally possible to ensure both that the plasma calcium remains normal and that there is a positive calcium balance. The failure is explained mainly by the difficulty in keeping the plasma phosphate within the normal range and in practice, it is often higher than is desirable because patients are often reluctant to take oral phosphate binding agents. Hyperphosphatemia is also harmful for other reasons, such as soft tissue calcification can develop and progress if the plasma calcium phosphate product is above a critical level. The normal individual, even in temperate latitudes, is able to maintain the sufficiency of vitamin D by means of solar exposure alone." @default.
