- W139075759 abstract "Obesity is a rapidly growing global epidemic. Every other woman of childbearing age is either overweight (body mass index, BMI, 24.9–29.9 kg/m 2 ) or obese (BMI 30 kg/m 2 or more) with a fifth starting pregnancy as obese. Obesity in pregnancy is associated with an increase in complications to the mother and baby. The management of obese mothers include appropriate identification and referral, regular surveillance for maternal and foetal complications and interventions to minimise weight gain in pregnancy and prevent adverse outcomes in current and subsequent pregnancies. A systematic review of effects of weight management interventions in pregnant women including obese and overweight mothers identified three main interventions for weight management in pregnancy: (1) diet and nutritional advice, (2) physical activity and (3) mixed approach incorporating diet and physical activity components underpinned by behavioural approach. The safety review of 26 studies (2 randomised controlled trials, 24 observational studies) involving 468,858 women showed that both diet and physical activity interventions recommended in clinical practice are safe. Diet-based interventions are the most effective in reducing gestational weight gain and maternal complications in pregnancy compared to other methods. Pregnant women who are obese are advised to increase their folate intake to 5 mg/day in the pre-conception period and first trimester to reduce the risk of neural tube defects. Vitamin D supplementation of 10 mg/day is recommended to prevent adverse outcomes associated with vitamin D deficiency. Obese women are at risk for venous thromboembolism, pre-eclampsia and gestational diabetes. The decision for antenatal and post-natal thromboprophylaxis in obese women will need to take into consideration other co-existent risk factors. A large preventive effect can only be achieved through public health and policy interventions that have an effect on the average weight of the society, which stops people tipping over the thresholds that define obesity. Thus, when focussing on obesity, it is important not to ignore the need for intervention in women who are not obese by definition." @default.
