En este ambito juega un papel importante la sustitucion artroplastica. El aflojamiento aseptico del componente acetabular es la complicacion mas frecuente e importante que se observa en el seguimiento a largo plazo de las artroplastias de cadera. Este aflojamiento no es exclusivo de las artroplastias cementadas, sino que tambien existe en las no cementadas, sobretodo en los modelos con cotilos roscados. A la vista de los resultados clinicos y radiograficos que se obtienen con los cotilos roscados, su utilizacion es debatida en la actualidad, pues proporcionan una buena estabilidad primaria pero no secundaria, con un elevado indice de aflojamientos. En el presente trabajo se realiza un analisis completo y exhaustivo sobre las causas y factores implicados en el aflojamiento aseptico de dos modelos de cotilos roscados : cotilo autorroscado de titanio CST y cotilo roscado de Mittelmeier (CM), ambos de Osteo(C). Para ello disponemos de dos series de pacientes, la primera integrada por 102 casos portadores de cotilos CST y la segunda integrada por 100 casos portadores de cotilo tipo Mittelmeier. Se realiza un analisis comparativo desde el punto de vista clinico y radiografico, ademas del correspondiente analisis de supervivencia. La evolucion de ambos modelos es relativamente similar, aunque el modelo CST es superior al modelo CM, debido a una mejor supervivencia estadisticamente significativa en funcion de la estabilidad y al hecho de que el resultado clinico evolutivo se ha mantenido descriptivamente en niveles superiores a lo largo de todo el seguimiento en el modelo CST respecto al modelo CM. No obstante, el elevado indice de aflojamientos observados ha llevado al abandono de su utilizacion. The aim of Orthopaedic Surgery in the field of articular pathology is trying to return the damaged joint its function. In this area artroplastic replacement plays an important role. The most common and important complication that can be seen in the long-term monitoring of hip arthoplasties is the aseptic loosening of the acetabular component. This relaxation is not unique to the cemented arthroplasties, but also exists in the cementless ones, especially in models with screwed acetabular components. Given the clinical and radiographic results obtained from screwed acetabular components, its use is currently being debated, since they provide good primary but not secondary stability, showing a high loosening rate. The following work develops a complete and exhaustive analysis on the causes and factors involved in aseptic loosening of two models of screwed acetabular components: titanium self-screwed acetabular component CST and Mittelmeier screwed acetabular component (CM), both by Osteo(C). This research has evaluated two sets of patients, the first consisting of 102 individuals carrying CST acetabular components and the second consisting of 100 individuals carrying Mittelmeier-type ones. In addition to the corresponding survival analysis, clinical and radiographic comparative analysis have been conducted. The evolution of both models is pretty similar, however the CST model has proven superior to the CM model. The reason for that is that the TSA model shows better significative statistically survival rates, related to stability and the fact that the CST evolutive clinical outcome has remained at higher levels than the CM one during all the surveyed period.
