- W1413763626 abstract "Objective : To in-vivo investigate whether differences in presynaptic dopaminergic activity exist between ventral and dorsal striatum (VST-DST) and how they impact on Parkinson’s disease (PD) clinical syndromes. Background: Neuropsychiatric disorders (NPDs) in PD are common but often misdiagnosed, even if are considered markers of patient’s quality of life. A degeneration of the dopaminergic systems might account for most of these manifestations. Noteworthy, the mesocortical and mesolimbic dopaminergic projections, arising from the ventral tegmental area and innervating the frontal cortex, the olfactory tubercle and the limbic system can play a crucial role. One previous MRI study reported atrophy of nucleus accumbens in PD. Another, investigating dopamine-reuptake and norepinephrine-reuptake sites with PET, found a decreased binding in the locus coeruleus and in the limbic system of depressed PD patients. A study, using PET dopaminergic measures, showed damage to ventral dopaminergic circuitry in subjects with PD genetic-risk. Methods: This study included N=17 patients diagnosed with idiopathic-PD (mean age 63.1 years, range [41-76], mean disease duration 3.7±4.3 years). Single-subject images were spatially normalized using a custom [ 11 C]FeCIT template built with N=11 healthy controls’ scan. In order to outline anatomical segregation between VST and DST, the former was functionally defined including nucleus accumbens, medial caudate and rostroventral putamen as described by Tziortzi et al., 2011. ROI uptake values were correlated with motor scores and evaluated with respect to presence of NPDs. Results: We found in PD patients: i)significant negative correlation between motor signs and striatal presynaptic dopaminergic uptake; ii)severe and selective presynaptic dopaminergic deficit in VST when we compared PD patients presenting depression and/or anxiety (DA+) with PD-DA - patients. Conclusion: Functionally segregated parts of the striatum in PD may differentially contribute to motor and non-motor symptoms.
