- W141509616 abstract "The ACC/AHA guidelines for intervention for mitral stenosis are shown in Fig. 1. Patients must have at least moderate mitral stenosis to warrant mitral replacement or balloon valvuloplasty. Patients also should be reasonably symptomatic with NYHA class III or IV symptoms. Other indications include significant pulmonary hypertension (peak pulmonary pressure >50 mm Hg) and/or recent onset atrial fibrillation. Patients with mitral valve morphology suitable for balloon valvuloplasty ideally should be referred for balloon valvuloplasty, given the good results and lower morbidity and mortality than mitral commissurotomy (2). An established echocardiographic scoring system is generally used to decide favorable valve morphology for balloon valvuloplasty, and it includes an evaluation of mitral valve leaflet thickness, calcification, and mobility as well as an assessment of submitral chordal fusion (3). Contraindications for balloon valvuloplasty include left atrial thrombus and more than moderate degree of mitral regurgitation. In the current era in the United States, mitral stenosis implies a high probability of mitral replacement, given that percutaneous balloon valvuloplasty is usually preferable treatment for mitral stenosis patients with valve morphology suitable for repair." @default.
