- W1419177229 abstract "1773 Objectives Cardiac MRI (CMR) and PET are established modalities for the diagnosis of cardiac sarcoidosis. However, it is not known whether they are competitive or provide complementary information. This study assesses the concordance of findings on CMR and PET in patients suspected of cardiac sarcoid that had both tests. Methods A retrospective chart review of seventy patients who underwent both CMR and PET for evaluation of suspected cardiac sarcoid was performed. Clinical and imaging findings were recorded including: a priori diagnosis of extra-cardiac sarcoid; ventricular function and wall motion abnormalities; presence and location of late gadolinium enhancement (LGE); presence and location of uptake of FDG on PET. Results 45/70 patients (64 %) had concordant findings on CMR and PET; either similar areas of LGE and FDG uptake, or no findings of sarcoid. 17 patients (24 %) had discordant findings on CMR and PET. In 8 patients (12 %) concordance could not be determined because the findings were equivocal. 25 patients without known sarcoid had concordant positive findings on CMR and PET. 6 patients with known extra-cardiac sarcoid had positive CMR and negative PET. Conclusions 64 % of patients suspected to have cardiac sarcoid had concordant findings on CMR and PET. Positive CMR was more likely to be associated with a positive PET in patients without known extra-cardiac sarcoidosis. The high rate of discordance (24 %) suggests that the modalities provide complementary information and the utility of PET would be enhanced if the decision to commence steroids and/ or tailor therapy is predicated on FDG uptake, particularly in patients unsuitable for CMR due to placement of a device. Cardiac sarcoidosis is a difficult entity to diagnose due to lack of consistent gold standard, thus necessitating amalgamation of disparate information from disparate modalities." @default.
