- W1425062017 abstract "Current assessment of erectile dysfunction often places excessive reliance on diagnostic testing, particularly on testing involving the intracavernous injection of vasoactive drugs. Such tests, especially cavernosometry and cavernosography, have not been validated by studies in control subjects and are associated with a high incidence of falsely abnormal results. Overreliance on these tests is often associated with taking of an abbreviated sexual history. In addition, nocturnal penile tumescence testing, which has been well validated by studies in control subjects, has been abandoned by many workers. In this era of the patient's goal-directed therapy, diagnostic testing should focus on the sexual and medical histories, physical examination, and blood studies to uncover associated systemic disorders. If nonsurgical treatment is elected, further diagnostic testing is usually not indicated. If surgical treatment is selected, psychogenic erectile dysfunction should be ruled out. The nocturnal penile tumescence test, when interpreted in light of the patient's sexual and medical histories, remains the best single test for differentiating psychogenic from organic erectile dysfunction." @default.
