- W1438034230 abstract "OBJECTIVE: Evaluate the effectiveness of a practice strategy aimed to reduce indwelling urinary catheter (IUC) placement in patients with acute ischemic stroke (AIS) who were hospitalized on a stroke unit at a Joint Commission certified primary stroke center. BACKGROUND: Urinary tract infection (UTI) is the most frequent nosocomial infection in patients hospitalized with AIS. IUC placement is a risk factor for UTI in these patients. DESIGN/METHODS: The IUC mitigation strategy incorporated a screening questionnaire designed to identify patients at risk for urinary retention, education of nurses and physicians in the emergency department and stroke unit about appropriate indications for IUC use, and clinical audits. Pre- and post-implementation data was collected via medical chart review. Data collected included placement and duration of IUC, and culture positive UTI. Data was analyzed with basic statistics. RESULTS: The pre-implementation cohort consisted of 217 consecutive AIS patients admitted over 1 year, and the post implementation cohort consisted of 112 consecutively admitted patients over 6 months. IUC was placed in 34 (16[percnt]) patients pre-protocol implementation and in 9 (7[percnt]) post-implementation (p=0.05). The duration of IUC placement was < 24 hours in 19 (56[percnt]) patients pre-implementation and in 8 (89[percnt]) patients post-implementation. Catheter associated UTI occurred in 10 patients (33[percnt]) in the pre-implementation cohort compared to no patients in the post-implementation cohort (p < 0.02). CONCLUSIONS: An interdisciplinary practice strategy focused on minimizing unnecessary IUC placement in AIS patients can reduce IUC placement in these patients. At our center, this intervention also resulted shorter duration of IUC placement in the stroke unit and a lower frequency of catheter associated urinary tract infection.
