• W1441285773 abstract "Zusammenfassung (Abstract) Die nosokomiale Pneumonie stellt mit 15-20% die zweithaufigste aller nosokomialen Infektionen dar. Trotzdem bleibt die Diagnostik dieser schwerwiegenden Infektion nach wie vor eine klinische Herausforderung. Trotz zahlreicher Studien gibt es bisher keinen diagnostischen Goldstandard der ventilatorassoziierten Pneumonie (VAP). Klinische, radiologische und auch mikrobiologische Kriterien weisen erhebliche Limitationen bezuglich der Sensitivitat, vor allem aber der Spezifitat auf. Die Folge ist meist eine antibiotische Ubertherapie, welche per se einen Risikofaktor fur die Entstehung einer Pneumonie darstellt. Um daher den diagnostischen Wert von QEAs bei der VAP isoliert und in Kombination mit bereits etablierten klinischen Scores wie z.B. dem Pugin Score zu untersuchen, beobachteten wir prospektiv uber den Zeitraum eines Jahres insgesamt 96 beatmete Patienten konservativer und operativer Intensivstationen der Heinrich-Heine Universitat Dusseldorf. Bei allen Patienten der Studie bestand initial bei Leukozytose, respiratorischer Verschlechterung, Hyperthermie oder positiver Radiographie klinisch der Verdacht einer VAP. Die definitive und standardisierte Sicherung der Diagnose erfolgte nach Aufnahme in die Studie orientiert an den Kriterien des „Centers for Disease Control (CDC)“. An Hand dieser CDC-Kriterien wurden die Patienten als „CDC-positiv“ (VAP +) eingestuft, wenn sie ein neues oder progredientes radiologisches Infiltrat sowie mindestens zwei der folgenden Kriterien aufwiesen: purulentes Sputum, Hyper-/ Hypothermie (>38.3°C, 12000/ml, 10 hoch 5 CFU /ml eine Sensitivitat von 0.86 und eine Spezifitat von 0.89 (ROC:AUC=0.87, ppW 0.84, npW 0.9). Bei Definition des Grenzwertes von >10 hoch 6 CFU/ml (>10 hoch 5 CFU/ml fur BALs) ergab sich zwar fur die QEAs eine erhohte Spezifitat von 0.96, zugleich aber nur eine Sensitivitat von 0.57 (ROC:AUC=0.77, ppW 0.95, npW 0.77); bei Grenzwerten von >10 hoch 4 CFU / ml (>10 hoch 3 CFU/ml fur BALs) ergab sich dagegen eine hohe Sensitivitat von 0.94, aber nur eine Spezifitat von 0.55 (ROC:AUC=0.77, ppW 0.59, npW 0.93). Bei den SQEAs (Grenzwert > masig) ergab sich eine Sensitivitat von 0.71 und eine Spezifitat von 0.82 (ROC:AUC=0.77, ppW 0.73, npW 0.81). Mit der Erhebung von QEAs (Grenzwert >10 hoch 5 CFU/ml) lasst sich zuverlassiger als mit SQEAs eine VAP bestatigen. QEAs sind auch Gram-Praparaten in Sensitivitat und Spezifitat deutlich uberlegen (Gram-Praparat allein: Sensitivitat 0.7 / Spezifitat 0.82; ROC:AUC=0,77; ppW 0,73;npW 0,81). Selbst eine Kombination von QEAs und Gram-Praparaten (QEA >10 hoch 5 CFU/ml kombiniert mit einem Gram-Praparat: Sensitivitat 0.87 / Spezifitat: 0.81; ROC:AUC=0,84; ppW 0,68; npW 0,95) er-hoht nicht die diagnostische Genauigkeit im Vergleich zu isoliert betrachteten QEAs. Unter Einbeziehung der QEAs in klinische Scores wie den Clinical Pulmonary Infection Calculation Score (CPIS-Sensitivitat : 0, 675%; Spezifitat: 0.964; AUC=0,82; ppW: 0,93 / npW: 0,81) nach Pugin lasst sich die Diagnose der VAP aber noch deutlich verbessern (Sensitivitat des modifi-zierten CPIS-m Scores: 0.9; Spezifitat: 0.839; AUC =0.871; ppW: 0.80 / npW: 0.92). QEAs stellen damit eine sinnvolle diagnostische Masnahme dar. Moglicherweise liesen sich mit Hilfe von QEAs Morbiditat und Mortalitat der VAP senken.
