- W1453245816 abstract "The demand for reversal of female sterilization has increased markedly in the last decade. However, advances in microsurgical technology have made this a reasonably safe procedure which can result in a pregnancy rate approaching 70%. Preoperatively, women should be counseled about what to expect from the surgery. The success of reversal is dependent on the type of sterilization procedure performed. Best results are obtained with the Pomeroy procedure and laparoscopic sterilization using clips or silastic bands. Procedures involving electrocoagulation of the tube have a lower success rate, depending on the amount of tube that was removed. The incidence of tubal pregnancy after microsurgical reversal techniques is estimated to be less than 1%. Preoperative evaluation should include a thorough history and physical examination. Where possible, the operative and pathology reports from the sterilization should be reviewed to estimate the amount and condition of the remaining tube. It must also be established if ovulation is possible. An appraisal of the fertility of the male partner is further recommended. An important surgical consideration is avoidance of postoperative adhesions. Systemic use of a dexamethasone-promethazine-antibiotic combination or intraperitoneal dextran are current regimens. Splints are no longer left in the tubes during the postoperative period since this has been shown to cause permanent damage to the tubal epithelium. Although the laser has a potential role in infertility surgery, it is not currently of significant benefit." @default.
