- W145687412 abstract "This chapter focuses on the various challenging aspects of antiphospholipid syndrome (APS) therapy, such as recurrent events despite optimal anticoagulation, long-term management of anticoagulation, the management of some rare nonthrombotic manifestations, the perioperative medical management of the APS patients. The chapter presents a case study of a 20-year old woman with the diagnosis of primary APS with new liver left lobe infarct. Four years prior to admission, the lady had developed right lower extremity arterial thrombosis, which was treated with thrombectomy followed by intravenous (IV) heparin and warfarin. In such cases, patients should be advised to stop smoking, have well-controlled blood pressure, and the women should be counseled against the use of estrogen-containing oral contraceptive pills. Patients should also introduce lifestyle changes to achieve and maintain ideal weight, cholesterol level, and physical activity. The APS patients who are undergoing renal transplant should be anticoagulated aggressively during the perioperative period. Recommended standard antithrombotic regimens for the high-risk patients should be the minimum administered dose in the APS patients. Timing of the anticoagulation is also crucial in APS patients to keep periods without anticoagulation to an absolute minimum." @default.
