- W1457978844 abstract "Since cervical mucus changes are not always consistent and complete enough to interfere with spermatozoa penetration and ascent they are thought important but not the sole explanation for the contraceptive effect of the low-dosage continuous progestogen method. This study describes morphologic modifications of gonads and oviducts apparently related to 4 of these agents. 33 women who applied for sterilization were included in the study. In Group 1 were 19 women who were on oral continuous low-dosage progestogen treatment. In Group 2 were 14 women who were using the same progestogen but on a precoital regimen. Laparotomy was scheduled according to the needs of the investigation while the subjects were under treatment. Macroscopic findings from both gonads and oviducts were carefully recorded. A wedge biopsy of 1 ovary was made in 23 of the women. The ovary and oviduct modifications were studied by light and electron microscopy. Conventional histology of follicular apparatus up to graafian follicle and theca luteinization was apparently normal. Corpora lutea were found in 14 subjects. 3-Beta hydroxy dehydrogenase activity was positive in some luteal structures. However luteal findings did not correlate with urinary pregnanediol excretion. In oviduct epithelium electron microscopy demonstrated interference with ciliogenesis microvilli Golgi apparatus and endoplasmic reticulum development. Lipid droplets and lysosomes (not observed in ovulatory controls) were found in endosalpinx cells. A direct relation was evident between tissue and cell modifications and clinical efficacy. These changes are reversible. It was shown therefore that oral progestogens induce modifications in cervical glands and endometrium that interfere with fertilization. They also modify the luteinization process and luteal structure functions. Rapid and significant modifications in oviduct epithelium are caused by continuous as well as precoital administration of progestogens. They are marked enough to assume interference with the transport and trophism of gametes." @default.
