- W1482413378 abstract "This article put forward by Kawasaki et al.1 demonstrates the feasibility of laparoscopic simultaneous bilateral adrenalectomies (LSBA). The authors report a series of 21 patients who underwent LSBA. Although conventional unilateral laparoscopic adrenalectomy is arguably a technically straightforward surgical case, LSBA is rarely performed, even when there are bilateral diseases present. As the authors pointed out, many of these patients undergo staged procedures. This may be due to the surgeons' perception that this sicker population of patients may not tolerate a longer surgery perioperatively. Further, the perception may be that postoperative management may be too risky when the patient is rendered adrenal free. The study of Kawasaki et al.1 demonstrates two noteworthy findings: (i) patients with bilateral adrenal pathology tolerate LSBA perioperatively with acceptable outcomes; and (ii) partial adrenalectomy in face of bilateral adrenal disease may be beneficial to obviate lifetime medical adrenal supplementation. With increasing evidence that LSBA is safe and efficacious, we owe it to our patients with bilateral adrenal pathology to take care of the problem in one setting! None declared." @default.
