- W1489582017 abstract "日本、美国、加拿大等发达国家,20世纪90年代就 开始利用低剂量螺旋CT进行肺癌筛查的研究,并与以前 的结果进行比较。Kaneko等报道了1975年-2002年分两 阶段进行肺癌筛查的结果,表明低剂量螺旋CT可提高 早期肺癌检出率。美国“早期肺癌行动计划(early lung cancer project,ELCAP)”报告:1 000名60岁以上吸 烟者中,低剂量螺旋CT非钙化小结节检出率为22.3%, 而胸部X线片仅检出0.7%,同时27例确诊为肺癌,使肺 癌手术切除率达96%。Tsushima等报道了低剂量CT肺 癌筛查的敏感性和特异性在结节阳性率方面分别为100% 和97%。初始诊断性高分辨CT(high-resolution CT, HRCT)在检测肺癌方面的特异性和敏感性为87.5%和 91.7%。并且认为,低剂量CT筛查出肺病变后,作诊断 性HRCT扫描,使肺癌筛查更合理,减少了不必要的跟 踪检测。国内,黄明刚等报道低剂量CT肺癌筛检的敏 感性和特异性分别为82.9%和88.8%,而<10 mm结节肺 癌诊断的敏感性和特异性为72.7%和89.6%。熊曾等用 Meta分析综合评价了低剂量螺旋CT筛查高危人群早期肺 癌的价值。通过检索国内外数据库,筛选文献,共纳入 15篇文献,由9个独立研究构成,共8 168例研究对象。 结论认为低剂量螺旋CT筛查高危人群早期肺癌基础扫描 中以发现结节直径≥5 mm为阳性标准较合理,总的诊断 效能较高,严密随访能提高筛查试验的准确性。目前, 已有大样本随机对照试验,进一步证实了低剂量螺旋CT 筛查早期肺癌的价值。因此,低剂量CT应用于肺癌的早 期筛查,具有重要的临床应用价值,对于早期肺癌的早 发现、早治疗都有切实可行的意义。" @default.
