- W1491230795 abstract "This chapter discusses various aspects associated with medication errors. Errors in taking the medication history can lead to prescribing errors. Failure to communicate essential information, transcription errors, and the use of drugs, formulations, or doses that were inappropriate for the individual patient are considered to be prescribing errors. Transferring patients from one type of care to another can result in medication errors. During 758 transfers of medications between primary and secondary care there were 142 medication errors, and on average there were two medication errors each time a patient was transferred. When patients were discharged from the hospital, the use of a specific medication dispensing system constituted a significant risk for medication errors. The most common error when patients were transferred to the hospital was the inadvertent withdrawal of drugs. On the other hand, when patients left the hospital the most common error was erroneous addition of drugs. The most common types of error throughout the medication process are: lack of drug form, unordered drug, omission of drug/dose, and lack of identity control. The methods that can be used to prevent or reduce the risk of errors are also reviewed." @default.
