- W1497017323 abstract "El objetivo del estudio fue evaluar la eficacia de la aplicacion de termoterapia en el perine durante el periodo expulsivo de parto normal para disminuir el dolor perineal y el numero de lesiones que necesitan sutura posparto.Metodo: Ensayo clinico multicentrico, abierto, con asignacion aleatoria al grupo de estudio: aplicacion de calor humedo (GCH), calor seco (GCS) y grupo control (GC). Se dirigio desde la Escuela de Enfermeria de la Universidad de Barcelona y se realizo en 5 hospitales adheridos al protocolo para la asistencia natural al parto normal del Departament de Salut de la Generalitat de Catalunya durante los anos 2009 y 2010. La muestra fue de 198 gestantes; a todas se les aplico el protocolo de parto natural y se asistio el expulsivo utilizando el cuidado habitual. Ademas en el GCH se aplico en el perine calor humedo mediante compresas mojadas con agua caliente y en el GCS mediante compresas colt-hot (nexcare®) calientes envueltas en una compresa seca. En el GC no se aplico calor. Durante el expulsivo se valoro el dolor en la zona perineal, mediante la escala de valoracion numerica (EVN de 0 a 10). Despues del parto se valoro el estado del perine. Se realizaron pruebas de contraste estadistico utilizando un intervalo de confianza del 95% y se realizo el analisis estadistico con PASW 17.Resultados: A los 10 minutos de aplicacion de termoterapia se constato una reduccion del dolor estadisticamente significativa: GCH un 27% (p<0,001) y GCS un 16,7% (p<0,001). Con referencia a las lesiones perineales posparto que no necesitaron sutura se obtuvieron un 15% mas en el GCH (p=0,070) y un 6% mas en el GCS (p=0,479) comparadas con el GC.Conclusiones: La aplicacion de termoterapia en el perine fue eficaz para reducir el dolor durante el periodo expulsivo de parto normal, pero no modifico el numero de lesiones perineales que necesitaron sutura posparto.Objective: To evaluate the effectiveness of the application of heat therapy to the perineum during the second stage of normal labour, in order to reduce perineal pain and the number of injuries requiring postpartum sutures.Method: Multicentre, randomized, controlled clinical trial with three study groups: application of moist heat (GCH), application of dry heat (GCS) and control group (CG). This study was directed from the Nursing School at the University of Barcelona and was conducted in five hospitals which apply the natural assistance protocol for normal delivery designed by the health department of the Catalan government for the years 2009 and 2010. The subjects were 198 pregnant women. The natural childbirth protocol was applied and the standard care during delivery was provided. In the moist heat group, heat was applied to the perineum with compresses wetted with hot water and in the dry heat group heat was applied with cold-hot compresses (Nexcare ®) wrapped in a dry compress. No heat was applied in the control group. Pain in the perineal area during delivery was assessed on a numerical rating scale from 0 to 10. After delivery, the state of the perineum was assessed. Statistical tests were performed with a confidence interval of 95% and statistical analysis was performed using the SPSS 17 package.Results: After 10 minutes of application of heat therapy to the perineum, the pain reduction was statistically significant: in the moist heat group pain decreased by 27% (p <0.001) and in the dry heat group by 16.7% (p <0.001). As regards postpartum perineal injuries did not require suture were obtained a 15% increase in the moist heat (p = 0.070) and a 6% increase in the dry heat group (p = 0.479) compared with the control group.Conclusions: The application of heat therapy in the perineum was effective in reducing the perception of pain during the second stage of normal birth, but did not affect the number of injuries requiring perineal suture postpartum" @default.
