It discusses the mechanisms that might be involved if a genetic factor is present. However, it does not present a proof of whether alcoholism is genetically influenced. The approach is to carefully match young men at elevated risk for the future development of alcoholism, based on their family histories of this disorder, with controls and test possible differences at baseline and after an ethanol challenge. The chapter briefly discusses the possible biological mediators of such a genetic factor. Thereafter, it presents the methods used in the investigations and go on to a brief overview of recent results. The investigations are carried out in two phases. The first step is to identify groups of individuals who are family history positive (FHP) and family history negative (FHN) and establish matched pairs. The second phase is to carry out actual testing of the two groups. One possible mediator of a genetic propensity in alcoholism could rest with genetic control of absorption and/or metabolism. This could relate to any single factor or the combination of the rate of absorption, absolute rate of metabolism of ethanol, rate of accumulation of acetaldehyde, rapidity of disappearance of this intermediary breakdown product, or accumulation of any of a variety of other metabolites. A second possible biological mediator of a genetic influence in alcoholism could relate to an individual's reaction to first few drinks in any evening that is individual's acute reaction to ethanol." @default.
