- W1509256276 abstract "閉塞性睡眠時無呼吸症候群 (OSAS) の一般的な治療は経鼻的持続的陽圧呼吸療法であるが, 本邦の睡眠呼吸障害患者の約30%には上気道疾患が存在し, 改善が期待できる場合は外科的治療も一つの選択肢である. 今回われわれは, 閉塞性睡眠時無呼吸症候群に対しオトガイ舌筋前方牽引術 (GA) と口蓋垂軟口蓋咽頭形成術 (UPPP) を施行し, その治療効果と合併症, および有効例と無効例に分けてその解剖学的な特徴について検討したので報告する. 対象は, 2006～2011年に当科を受診し中等症以上の OSAS 患者と診断された24例である. 評価項目は, ESS, イビキ・熟睡感, AHI の改善度, 睡眠構築の変化, 術後合併症およびセファロメトリーを用いて有効例と無効例を比較検討した. 24例中16例 (66.7%) で AHI の改善を認めた. AHI の改善がなかった1例は, その後, 上顎骨・下顎骨切離前方転移術を施行し AHI の改善を認めた. 有効例は, 上顎および下顎後退が軽度でさらに軟口蓋過長がみられない症例であった. GA と UPPP の手術適応は, SNA>79.11°, SNB>75.69°, FX>78.67° および36.79mm<PNS-P<42.49mmであった. 保存的治療が増える傾向がある昨今ではあるが, 閉塞部位が明確な場合は顎顔面手術も有効な治療法であると思われた." @default.
