- W1511091630 abstract "For decades oral vitamin K antagonists (VKAs) have been the therapy of choice for long-term anticoagulant therapy in patients at risk of venous or arterial thromboembolism. In recent years several oral direct-acting inhibitors of activated coagulation factors have been tested and some introduced for therapy in several different disease conditions. The direct thrombin inhibitor dabigatran etexilate and the factor Xa inhibitors apixaban and rivaroxaban have been registered for use in orthopedic surgery, and rivaroxaban also for the treatment of deep vein thrombosis. Rivaroxaban and dabigatran etexilate have also shown promising results in the treatment of pulmonary embolism as well as for long-term prophylaxis after venous thromboembolism. These three medications have also been found either non-inferior or superior to VKA for stroke prevention in patients with atrial fibrillation where they are associated with a lower risk of hemorrhagic stroke than VKA. In patients with acute coronary syndrome the use of VKA or the new anticoagulants on top of dual platelet inhibition is associated with a substantially raised risk of bleeding, but only with slight reduction in ischemic events." @default.
