- W1513791036 abstract "Publisher Summary This chapter focuses on the normal and abnormal structure of human nociceptors, aspects of their normal and abnormal afferent sensory and efferent neurosecretory functions, and some interactions between nociceptors and the sympathetic system. In addition, two specific pathophysiologically based syndromes of nociceptor dysfunction are described and contemporary attribution of hypothetical secondary central somatosensory dysfunction to hypothetical chronic dysfunction of primary human nociceptors is critically analyzed. The idea of secondary centralization of pain mechanisms rests mainly on failure to detect a sufficient peripheral explanation for subjective or psychophysical neuromuscular symptoms and signs in patients who in reality lack organic disease. In turn, on human experimental grounds, the centralization hypothesis rests largely on failure to detect sensitization of primary nociceptors as a substrate for secondary hyperalgesia induced by algogenic substances. At present, industries engaged in therapeutic endeavors for chronic neuropathic pains focus on destroying the sympathetic nervous system and blocking excitability of second order pain-signalling neurons assumed to be decisively sensitized." @default.
