- W1523771549 abstract "Over the last decade, it has become clear that many patients with Parkinson's disease (PD) or diffuse Lewy body dementia develop REM (rapid eye movement) sleep behavior disorder early in the disease process. This mysterious disorder, in which patients essentially act out frequently violent dreams, can cause injury to the patient or the bedpartner. In this chapter, we discuss the brain circuitry that controls sleep–wake cycles and transitions to REM sleep. We identify the sites in the brainstem that are responsible for eliminating muscle tone during normal REM sleep, and why these fail in REM sleep behavior disorder, thus allowing the patient to be active during dreaming. In addition, we discuss how this same circuitry is involved in control of motor tone, and its implications for the postural and gait disorders of PD." @default.
