The experience with sensitivity testing in human immunodeficiency virus (HIV) infection may bear implications for other chronic viral infections. Loss of helper T lymphocytes and destruction of lymphatic tissues are the major pathological sequelae of infection with the HIV. They ultimately result in the development of the acquired immunodeficiency syndrome (AIDS). Resistance to antiviral therapy reflects the ability of HIV to replicate in the presence of anti-HIV drugs, and this is associated with an accelerated disease progression. Acquired resistance to chemotherapy has been a challenge with both bacterial and parasitic infectious diseases for some time. Mechanisms for avoiding the development of therapeutic failure due to resistance, such as maintenance of sufficiently high drug levels and combination therapy consisting of two, three, or more drugs at a time, are being employed for the treatment of these infections. Several methods of drug sensitivity testing are routinely used for the clinical management of bacterial diseases. Assays, such as the disk diffusion, the broth dilution, and the E test are quick, relatively cheap, and simple to perform. At present, two enzymes––the reverse transcriptase (RT) and the protease (PR)––are targets for antiviral therapy of HIV infection." @default.
