Accordingly, the types of deceased donors considered suitable for organ donation continue to be expanded to include donors whose death is due to cardiorespiratory collapse rather than brain death. These donors are categorized as providing donation after circulatory death (DCD), and sufficient experience has been gained with organs from these donors to recognize them as clearly viable alternatives for use in organ transplantation. However, that experience has also uncovered aspects of DCD organs that distinguish them from living donor or brain dead donor organs, and a thorough understanding of these issues is required to make appropriate use of this source of donor organs. This chapter will review acceptance criteria for DCD organs, discuss the organ-specific risks and benefits of DCD transplantation, and delineate short- and long-term outcomes after DCD transplantation.
