Elle est peut-être utile très précocément, avant la constitution d'une insuffisance rénale aiguë. Elle peut être dangereuse en cas de plaie opératoire. La situation peut également être aggravée par les antifibrinolytiques. Endotoxin acts on hemostasis at many levels : under its influence endothelial cells enter the circulation, platelets release and aggregate, synthesis of factor VIII and of fibrinogen is increased, hepatic clearance of F.D.P. (fibrinogen degradation products) and of certain activated factors is decreased. This leads to D.I.C. (disseminated intravascular coagulation) with drop of platelets and of factor V, presence of F.D.P. and fibrin monomer, lenghtening of thrombin and reptilase times, and positive protamin test. Activation of the coagulation system and particularily of factor XII induces an activation of the kallikrein-kinin system, which most likely plays an essential part in the genesis of shock. Heparin must be used if it is necessary to correct fibrinogen and/or platelet levels by the use of exogenous fractions. It may also be useful at a very early stage, to prevent renal capillary thrombosis. However heparin can be dangerous in surgical patients. Use of antifibrinolytic drugs can also be dangerous during D.I.C." @default.
