- W1542004 abstract "Background: Before starting the cooled flush perfusion of explantation-lungs a prostazycline bolus is given via the pulmonary artery with the aim to prevent a vasoconstriction during flushing. When the perfusion pressure is too high, there is a risk for the development of a lung edema. The aim of our study was to examine if an inhalative applicated prostacycline (PGI2) protects the lungs better against edema formation during flushing in comparison to the other application form. Method: The pulmonary artery of 24 isolated, mechanically ventilated porcine lungs was perfused with low-potassium dextrane solution (+4°C) at a hydrostatic perfusion high of 65 cm (PAP mean=25 mm Hg). During flushing the lungs were ventilated with a lung protection ventilation (VT=6ml/kg, frequency 12/min, PEEP=8 cmH2O). We randomized all lungs into 3 groups (n=8 for each group): group 1: no application of a prostacycline, group 2: bolus application of a prostacycline, group 3: inhalative application of a prostacycline. The lung’s weights, compliances and histopathologies were assessed before and immediately following perfusion and after 4 hours of cold storage. Results: Following perfusion and storage, the relative increases in lung weight were smaller in the inhalative and the control group versus the bolus group (p<0.05). Compared to the bolus groups the inhalative group was the better looking and the lung histology appeared to be least affected after flush perfusion and cold storage. Conclusion: Our findings suggest that lung edema formation during flush perfusion in ventilated isolated lungs can be reduced by using inhalative prostacycline." @default.
