- W1543464995 abstract "Traumatic rupture of the aorta, including complete transection, is occurring with increasing frequency as modern vehicles carry an enlarging number of people at progressively faster speeds. This type of injury usually occurs in a setting of rapid deceleration and should be suspected after any such accident. Although many of these patients die immediately, a significant number reach the hospital alive. Only by rapid recognition and delineation of the injury and expeditious repair can these patients be saved. Widening of the superior mediastinum in the appropriate clinical setting should arouse suspicion of such a lesion, and definitive diagnosis with localization of the site of injury is accomplished by angiography. Therapy then consists of institution of appropriate extracorporeal circulation and establishment of proximal and distal aortic control prior to entering the surrounding hematoma, followed by use of a graft to re-establish aortic continuity. Two patients with complete transection of the thoracic aorta at the isthmus successfully managed in this manner are reported." @default.
