- W1543598968 abstract "Ultrasound screening in perimenopausal patients reveal a suspicion of endometrial hyperplasia in some of them. Women with endometrium thickness of more than 5 mm are usually referred for endometrial biopsy to confirm the diagnosis. In some of them, despite ultrasound image of wide endometrium, endometrial atrophy is diagnosed in pathological examination. This group of patients was analyzed to assess if elastography is able to differentiate atrophic endometrium and endometrial pathology like hyperplasia, cancer or polyp. Elastography images of the uterine cavity of 35 perimenopausal patients referred for dilation and curettage (D&C) due to endometrial thickness of more than 5 mm, with no clinical symptoms, were described by Elastography Index (EI). EI was correlated to the pathological findings. The endometrium images of all the patients with endometrial pathology (hyperplasia, cancer, polyp) were assigned 2 or more EI points (14 patients), and in all the patients with normal or atrophic endometrium (21 women) - 0 or 1 point. The difference was significant (χ2 Pearson test; P < 0.0001). In this small group, the sensitivity of the Elastography Index of endometrium was 100%. However, the bigger group of patients has to be analyzed to confirm this observation and thus, to change the management of patients with the endometrium thickness of more than 5 mm but with EI below 2 points leading to avoid unnecessary endometrial biopsy in these women.
