- W1550126900 abstract "HistoScanning has been introduced to the market with only a few publications showing its potential accuracy in detecting prostate cancer (PCA) foci, and its technique has been questioned.1 Three2-4 out of the five2-6 existing original publications with a limited number of patients have shown the potential visualization of PCA foci within the prostate in PCA patients before radical prostatectomy. The HistoScanning technique seems to work to some extent for the aforementioned reasons. The question should be raised as to what benefit HistoScanning adds to our daily urological work. Just two articles deal with the clinical issues of: (i) the value of HistoScanning for targeted biopsies;5 and (ii) the prediction of a negative margin in prostatectomy patients.6 The current article by De Connick et al.7 shows a potential role for HistoScanning in selecting patients in whom prostate biopsies are necessary. To date, we are generous in the decision-making for a biopsy in the case of an elevated prostate-specific antigen and/or positive digital rectal examination, and are willing to rebiopsy patients in whom biopsies were negative, but there is a persisting suspicion. Therefore, an objective tool to decide whether or not to biopsy or to avoid biopsy is required. The authors concluded that in the case of a total lesion volume of <0.5 in the Prostate HistoScanning (PHS), a biopsy might be avoidable. In addition, a 4.5-fold higher chance for a positive biopsy with a HistoScanning-guided biopsy compared with a random biopsy was described in the examined patients. The better per core detection rate for the PHS-guided biopsies is an interesting result. However, one might question if the expense alone of such a technique can justify its use, and if additional cases of prostate cancer will be detected that might otherwise have not been detected by random biopsy. In my eyes, the question of whether HistoScanning can securely avoid biopsies cannot be answered by the present study. The number of patients is relatively low and the follow-up time is not long enough. The ideal setting to obtain the real negative predictive value would be in patients who undergo a cystoprostatectomy. Nevertheless, the authors can be congratulated for initiating the study to shed more light on the potential clinical value of HistoScanning. Finally, HistoScanning has one thing in common with most of the other innovative imaging methods, such as multiparametric magnetic resonance imaging, elastography, computerized transrectal ultrasound or contrast-enhanced ultrasound: data are sparse, and multicentric studies to show the real impact of these methods on the patients are missing.
