- W1557605184 abstract "BOOK REVIEWS Evans' Histological Appearances of Tumours. 2 vols. By David J. B. Ashley. 3d ed. New York: Churchill Livingstone, 1978. Pp. 857. $85.00. Think of the mammoth undertaking of compiling an up-to-date, complete compendium of tumor pathology which itemizes each important gross and microscopic feature of each benign and malignant human tumor and discusses their cell or cells of origin; how differentiation, metaplasia, or dedifferentiation might alter their cellular appearance; and, finally, how each spreads and what are their individual prognoses. Of course there should be a complete bibliography which identifies the important papers written about each tumor, and this listing should emphasize the more recent reports. It is no wonder that texts of tumor pathology are so few. Willis's Pathology of Tumours (1948) and Evans's HistologicalAppearance ofTumours (1956) are the sum total available in the English language. The Evans was still incomplete when it was first published, without coverage of the gynecological tumors, but the second edition remedied that deficiency. Overall texts of tumor pathology must compete with specialty texts covering single anatomic areas, like pathology of the lung, skin, or genital tract. Despite the awesome task outlined here, a new, handsome, and greatly rewritten double-volume edition with a larger format ?? Evans' Histological Appearance of Tumours has been published. Each chapter begins with the embryologie origin of the tissue from which the tumors are derived, and then normal histology is described. This gives the reader a good background on which to base an understanding of the differential features of hyperplasia, adenoma, and carcinoma to be described later in that chapter. What exactly is a textbook of tumor pathology supposed to accomplish? The response might be simply to teach the reader what the morphologic features of each tumor are that allow it to be specifically diagnosed. Unfortunately, there are not pathognomic features identifiable in every separate tumor occurring in man. So one must include a listing of less specific features, which in their totality may lead to the correct diagnosis. I consider it exceedingly naive to think that all tumors can be identified based on verbal descriptions. In fact, it can be argued that in some instances we do not know what features lead to a specific diagnosis. Pathologists are always saying, It is an adenoma because it looks like an adenoma. When you hear yourself saying that you know you have failed in your responsibility to teach morphology, and you search wildly for some morphologic configuration to back up your diagnosis. In a morphologic text such as Evans' Histologic Appearance of Tumours, numerous photomicrographs supplement the verbal descriptions and convey to the reader morphologic observation unPermisssion to reprint a book review printed in this section may be obtained only from the author. Perspectives in Biology andMedicine ¦ Winter 1980 | 311 encumbered by translation into verbal text. These photomicrographs are a strong asset to the total purpose of this book. The author, David Ashley, a student of Winston Evans's, has tried to carry forward the strong embryologie flavor of the two original editions of this book. Evans's contributions to tumor pathology were from the dual vantage point of a trained embryologist and pathologist. This adds an extra dimension to pure morphology, which often explains variations in structural characteristics which would otherwise remain unexplained. The greatest deficiency of this well-conceived, well-presented, double-volume text is the lack of emphasis on the most recent literature, both in the text and in the bibliographic listings at the finish of each chapter. The overall balance is well maintained between tumors of different tissue origin. Due to the author's preference , the reproductive-tract neoplasms probably get a little more thorough handling, while tumors ofthe lymphohematopoetic system are relatively slighted in this presentation. If you have not already guessed, I am a long-time booster of R. Winston Evans's embryologie-morphologie analysis of human tumors, and I find the current David Ashley authored third edition a worthwhile addition to the library of any scientist who needs to interpret the vagaries of human tumor behavior.
