- W155772580 abstract "Alcohol consumption and cigarette smoking are likely to be differentially associated with the risk of head-neck cancer (HNC) subtypes (i.e. oral cavity cancer (OCC), oro-/hypopharyngeal cancer (OHPC) and laryngeal cancer (LC)), but few prospective studies were able to examine this. Therefore, we investigated these associations within a large-scale prospective cohort study. 120 852 participants completed a questionnaire on diet and other cancer risk factors at baseline in 1986. After 17.3 years of follow-up, a total of 395 HNC (110 OCC, 83 OHPC, and 199 LC) cases and 4288 subcohort members were available for case-cohort analysis using Cox proportional hazards models. For all HNC, the multivariable adjusted incidence rate ratio (RR) was 2.7 (95% confidence interval (CI) 1.8–3.9) for those drinking ⩾30 g ethanol/day compared with abstainers; in subtypes, RRs were 5.8 for OCC, 3.5 for OHPC, and 1.5 for LC. After adjustment for total alcohol intake, we found no significantly associated risks between intake of beer, wine, liquor and any type of HNC. Current smokers compared with never smokers had a RR of 4.5 (95% CI 3.1–6.5) for HNC overall, and 2.1 for OCC, 8.5 for OHPC, and 8.1 for LC. Smoking duration appeared to be of greater influence on HNC-risk than smoking frequency. A positive, multiplicative interaction between alcohol consumption and cigarette smoking was found for HNC overall (p interaction 0.02). Alcohol consumption and cigarette smoking were independently associated with risk of HNC overall, with a positive, multiplicative interaction; the strength of these associations differed among HNC-subtypes. Of all subtypes of HNC, OCC was most strongly associated with alcohol consumption, but most weakly with cigarette smoking. The differential associations between alcohol consumption, cigarette smoking and HNC-subtypes are likely due to diverse effects on anatomic subsites in the head-neck area." @default.
