Die Behandlung und auch die Prognose hangen stark von der Operabilitat, Lokalisation und anderen Faktoren, wie z.B. MGMT-Status, ab. Andere konventionelle bildgebende Verfahren, wie MRT oder CT, konnten bislang, insbesondere nach Therapiebeginn aufgrund von unspezifischen Entzundungsreaktionen, keine sichere Aussage bezuglich Tumoraktivitat und Therapieansprechen treffen.Das vorrangige Ziel bestand daher unter anderem darin, durch nicht-invasive Masnahmen den Krankheitsverlauf des Patienten im Voraus einschatzen zu konnen, um hierdurch eine Therapie speziell an die Bedurfnisse des Patienten anzupassen, aber auch darin, eine bereits begonnene Therapie auf ihren Erfolg uberprufen zu konnen.Anhand der Daten durch FET-PET-Untersuchungen zu verschiedenen Zeitpunkten innerhalb eines Therapieprotokolls wurden 79 Patienten auf eine Korrelation zwischen Volumen, SUVmax/BG bzw. Kinetik und Gesamtuberlebenszeit bzw. progressionsfreier Zeit untersucht. Es stellte sich heraus, dass alle drei untersuchten Marker (SUVmax/BG, Volumen, Kinetik) in der Eingangsuntersuchung geeignete Marker sind, um sowohl die progressionsfreie Zeit als auch die Gesamtuberlebenszeit bei einem Patienten einschatzen zu konnen. Hier erscheint insbesondere das BTV vor Radio-Chemotherapie interessant, da es mit PFS und GUZ korreliert. Ebenso konnte gezeigt werden, dass nach Therapiebeginn die Untersuchung der Kinetik ein hervorragender Marker ist, um prognostische Aussagen bezuglich des weiteren Krankheitsverlaufs treffen zu konnen. Ein Therapieansprechen und, im Falle einer Operation, ein Resttumor konnen erkannt werden.Zusammenfassend lasst sich sagen, dass die additive FET-PET einen festen Stellenwert in der Diagnostik und der Beurteilung des Therapieansprechens bei Glioblastoma multiforme haben konnte, da mit den hier untersuchten Markern bessere Ergebnisse erzielt werden konnten als mit den konventionellen Standardverfahren." @default.
